Voice/Telephone

Repair of U.S. Cellular phones and accessories

Repair requests will only be accepted from previously approved departmental staff whose names are on file with DoIT Voice Services. If you don't know who this is, please contact your departmental financial specialist or purchasing personnel for assistance. For further questions, call DoIT Voice Services at 262-5544.

Fill out the Repair Form and press Submit when completed.

Once the form is received at U.S. Cellular

  • A working phone with battery and AC wall charger and/or accessory will be shipped within 2 business days.
  • For State of Wisconsin owned equipment (non-Nokia 5165, 3586i, 6019i, or LG UX145 equipment), a LOANER will be shipped. (When State of Wisconsin owned equipment is repaired, the ship exchange process must be used to return the loaned equipment.)
  • If there are repair charges for out of warranty or use damaged items, the customer will be called first before chargeable repairs are initiated.

Once equipment is received at the user's location

  • Call 800-819-9373 to activate your replacement phone.
  • Place the defective phone with battery and AC wall charger and/or accessory in the box in which the working equipment was shipped, affix the enclosed shipping label, and send to the address on the label.

    Note: You may want to clear your call log and contact list to prevent sharing of any sensitive information.
