Ordering Cellular Service
After you've selected your cellular equipment, follow the ordering instructions below. For authorization purposes, email requests will only be accepted from previously approved departmental staff whose names are on file with DoIT Voice Services. If you don't know who this is, please use this form to obtain that information.

U.S. Cellular Replacement/Upgrade Cellular Equipment Order Form
Use this form to replace existing cell phones under the terms of the new contract.

Use this form to order new cellular service and equipment.
Verizon Wireless New Broadband Data Card Order Form
Use this form to order new cellular broadband data card service.
If you are ordering an alternate phone (not the free phone on contract) and/or accessories, all charges will be added to your monthly TAR statement.

Changing funding of cellular service
If you wish to change the fund/account for cellular service, complete the Telephone Funding Change form.

Disconnecting service
To disconnect cellular service, send an email to voice.services@doit.wisc.edu with the cellular telephone number (see above authorization requirement). Note: Clear your call log and contact list to prevent sharing of sensitive information before turning in the cellular phone.
Donating state-owned cell phones
Cell phones should not be donated! Donating cell phones can leave the State open for liability issues. If there is still service when the phone gets into someone else's hands, the phone service can be used and charges will apply and be billed to the original user. This could mean high charges on a UW-Madison department's telephone bill for calls that someone else has made.
UW SWAP (497-4440) can help you properly dispose of cell phones. When sending old cell phones to SWAP, please include the original box, if available, manuals, extra batteries and accessories. Each department is responsible for deleting all data and de-activating the phone with the cellular provider. SWAP does not do this.
