Ordering Cellular Service
After you've selected your cellular equipment, follow the ordering instructions below. For authorization purposes, requests will only be accepted from previously approved departmental staff whose names are on file with DoIT Voice Services. If you don't know who this is, please contact your departmental financial specialist or purchasing personnel for assistance.
Ordering U.S. Cellular Service and Equipment
Cellular Equipment REPLACEMENT Order Form
Use this form to replace existing cell phones under the terms of the new contract.
NEW Cellular Service and Equipment Order Form
Use this form to order new cellular service and equipment.
If you are ordering an alternate phone (not the free phone on contract) and/or accessories, payment is required either by P-Card (U.S. Cellular’s preferred method) or by external requisition. For P-Card users, U.S. Cellular will call you for credit card information. If you are using an external requisition, follow the procedures below.
External Requisition - use the on-line requisition generator instructions.
Complete an external requisition for the purchase of cellular equipment ONLY if something other than the free contract equipment is to be purchased (including accessories). Send this external requisition through the normal routing procedure for external requisitions.
- This external requisition must include a complete description of the equipment to be ordered with associated costs and should be addressed to:

Madison WI 53707-7835 - The Requisition must include:
- Type and number of telephones
- Accessories if needed
- Current telephone number
- User’s name
- Department name
- Delivery address
- The contact name on the external requisition should be the same as the person ordering the service. This contact information must include the name, Centrex telephone number and campus mailing address. If the equipment is to be shipped to the user of the cellular telephone, specify this person's name, Centrex telephone number and campus mailing address.
- Equipment will be shipped about two weeks from the date that UW-Madison Purchasing Services receives the signed external requisition.
- Your telephone will be active upon arrival. The telephone number will be on the packing slip and will also be retrievable on the telephone by pressing the RCL button, then the # button.

Changing funding of cellular service
If you wish to change the fund/account for cellular service, complete the Telephone Funding Change form.
Two Cell Phone Numbers on One Cell Phone
Dual NAM allows two cell phone numbers from the same cellular vendor to be associated with one phone.
Disconnecting service
To disconnect cellular service, send an email to voice.services@doit.wisc.edu with the cellular telephone number (see above authorization requirement). Note: Clear your call log and contact list to prevent sharing of sensitive information before turning in the cellular phone.
Donating state-owned cell phones
Cell phones should not be donated! Donating cell phones can leave the State open for liability issues. If there is still service when the phone gets into someone else's hands, the phone service can be used and charges will apply and be billed to the original user. This could mean high charges on a UW-Madison department's telephone bill for calls that someone else has made.
UW SWAP (497-4440) can help you properly dispose of cell phones. When sending old cell phones to SWAP, please include the original box, if available, manuals, extra batteries and accessories. Each department is responsible for deleting all data and de-activating the phone with the cellular provider. SWAP does not do this.
