Cellular Policies

Converting UW-Madison Cellular Service to a Personal Account

If you’d like to convert your UW-Madison-provided U.S. Cellular phone to a personal account, please read on for more information:

  1. You may keep your cellular number.
  2. You must select a new consumer rate plan (no part of the State contract will apply).
  3. You must sign a new Customer Service Agreement/Contract, taking billing and liability to your home address.
  4. You will be treated as an existing customer so you will not be eligible for promotions offered to new customers (free or discounted cell phones, etc).
  5. You must obtain a new cellular telephone. Your current cellular phone may be owned by U.S. Cellular or a campus department. U.S. Cellular employees will make that determination when they review your phone at the retail store. If the cellular phone is owned by U.S. Cellular, U.S. Cellular staff will retain the phone at the time of contract signature. If owned by a campus department, you must return the phone to the appropriate department for re-use or disposal via SWAP.
  6. Transaction takes place at U.S. Cellular's East or West side retail stores.

For authorization purposes, email requests will only be accepted from previously approved departmental staff whose names are on file with DoIT Voice Services. Voice Services will send you a Change of Liability Request Form. You should present the signed Change of Liability Request form, a copy of this document, and the current cellular telephone to a U.S. Cellular employee at one of the following retail locations:

U.S. Cellular will return a copy of the signed Change of Liability Request Form to DoIT Voice Services via fax (265-6767) or mail (1210 W. Dayton St., Madison, WI 53706).
