ASG-Cypress Repository Application Form

Follow the steps below to submit your computer-generated report for presentation on My UW-Madison.

Step 1: Prepare an abstract of the computer-generated report and explain why you want your report presented in My UW-Madison. To do this, first copy your abstract to your desktop and then submit it by email to mumcypressrpt@lists.wisc.edufor review by the My UW-Madison Advisory Group. After review, you will be notified to complete Step 2.

Step 2: After you receive Advisory Groups approval, complete the form below to provide delivery information. Submit the completed form via email to mumcypressrpt@lists.wisc.edu.

1. Requestor name

2. Requestor department office name

3. Requestor email address

4. Requestor phone number

5. Report title

6. Source application system (Payroll, etc.)

7. Job name (if known)

8. Report generation schedule (e.g., Fridays, end of month, etc.)

9. Report generation frequency (daily, weekly, etc.)

10. Cypress database retention duration (days)

11. Target recipients (student, faculty/staff, etc.)(Only for those with access to the Madison My UW)
(describe in system terms)

12. Unique recipient identifier (ID) for report delivery

13. Report developer/technical support contact name (if known)

14. Report developer division, department

15. Can you provide test data for development of the application and for other testing purposes? yes no

16. Preferred target date for initial delivery

17. Will data contain sensitive information that will be displayed in the portal? - If YES, what is the information? Ex: SSN, PhotoID#

18. Purpose of this request?

Step 3: Using the form you submit and other information, the ASG-Cypress Server Page (CSP) service provider convenes a technical team to assess development requirements. In assessing requirements, the technical team considers such questions and issues as:

  • Are there any known privacy or security considerations (SSN#)?
  • Does the application require the use of forms overlay? What information needs to be displayed on the form?
  • How will the data be sent to ASG-Cypress?
  • What indexes are needed to identify the application in MUM and/or reprint purposes?
  • Are there any special requirements for look and feel within MUM?
  • Does the online form need to match the hard copy exactly?
  • How do you plan to handle accessability issues (print instructions, contact person etc.)?

Next, a customer contact team meets with you to review requirements, consider any adjustments needed in the design specification, and resolve any outstanding issues. These issues are addressed in the meeting: Test Plan (accessibility, browser, user acceptance)

  • Communications (Customer provides text for MUM announcements, "What's New", etc.)
  • DoIT Help Desk and customer office support plan, including any FAQ's
  • Mechanism for approving the load of production data (e.g., RMS hold queue with approval or denial)
  • Agreement on estimated development and storage costs
  • Agreement on delivery presentation within the portal
  • Agreement on a delivery timeline
  • Agreement on ongoing maintenance of reports within the portal. Customer must agree to notify ASG-Cypress system technologist if data fields change, so that index field parameters can be update
