Service Request Form

Repair and Departmental Support offers both on-site repair and pick-up & delivery service for computers and servers. For printers we offer on-site repair (HP LaserJets only).

On-site Repair

For a $35.00 charge, Repair and Departmental Support will come to your departmental location to perform on-site repair. Dorms, apartments, and homes are NOT included. This fee does NOT apply to printers, for which onsite repair is required.

Pickup & Delivery Service

Repair and Departmental Support will pickup and deliver equipment to any campus departmental location free of charge. Dorms, apartments, and homes are NOT included.

Please call (608) 263-7237 to provide payment information if paying with either a department Visa card or a personal credit card, and request to have a receipt of charges sent to the email address of your choice. You will NOT be charged until work is complete.
