Access to UDS Questionnaire

  1. What office is requesting this data? Who will be directly responsible for the account and the security of the data that is accessed via that account?

  2. What do you plan to do with the data? (Share it? Store it? Crosswalk?) What data elements are you requesting?

  3. What is the extent of people downstream from you that will have access to the data (i.e. will only people in your office be able to see the data or (e.g.) will people on campus that you serve also have access)?

  4. At what frequency should the data be refreshed (how current does the data need to be)?

  5. What is the preferred method of data collection? (MAADA, LDAP, Oracle View, SQL, batch file).

  6. What type of account should we set up; a personal account, or proxy? (If an actual person is logging on, they should use a personal ID and password. If the intent is to build an application where the ID and password must be stored within the application, then a proxy ID should be used.)

  7. How soon do you need to have this access?
